The purchase order form provides customer information, shipping address, salesperson, payment Type, and the price and quantities of items ordered. A wraparound back cover Tucks between pages so you don't write through multiple sets. The blank space in the header Leaves room for a company stamp, and the unique 6-digit receipt numbers help you easily locate orders.
Returns
Domestic USA Orders Only.
We accept returns on items for a period of 30 days from the date of purchase. All product and packaging must be in new and unused resalable condition. Shipping is non-refundable.
To initiate a return, please contact us for an RMA number. RMA's can take up to 2 business days to generate. Returns without an RMA number will not be accepted.
