This is a standard version of a
Living Will Declaration / Advance Directive Form
which is non state specific.
You can edit the form based on your particular needs.
There is a clause at the end which needs to be signed and notarized for legal purposes. The form is a total of 2 pages in length and will be
emailed
to you in a file within 24 hours of receiving payment.
You will need to have a word program and a printer to open and print this form. A hard copy can be mailed for an additional fee to cover postage and handling.
